Reason for disclosure: The (Client Name) has engaged SOTERA Investigative Group, LLC to conduct backgrounds inquiries in connection with your application or candidacy for employment with the Client. The background checks may include those listed below. The information collected from you and about you by SOTERA Investigative Group, LLC will be kept strictly confidential and will be obtained for and disclosed only to the Client or, to others only upon your express consent or as may be required or permitted by law. The information is requested to assist the Client to determine your qualifications and suitability for employment. I Consent to perform the different searches and to collect the personal information requested by signing below; i) I authorize the background checks to be conducted as a condition of employment; ii) I certify that the information I have provided on the SOTERA Investigative Group, LLC consent forms, attached and on my resume and/or application form, is complete and accurate in every respect. I understand that SOTERA Investigative Group, LLC will validate the information provided.
I authorize SOTERA Investigative Group, LLC to contact the third parties I identify for the purpose of performing these background checks. I understand that SOTERA Investigative Group, LLC will hold all personal identification information collected from me in order to perform the background checks confidentially and consent to the release of that information to third parties (other than the Client) as necessary for the completion of SOTERA Investigative Group, LLC reports.
I understand that SOTERA Investigative Group, LLC is acting as agent of the Client. I authorize SOTERA Investigative Group, LLC to release all personal information obtained in the course of the background checks about me to the Client and in no way will hold SOTERA Investigative Group, LLC or the Client liable upon the release of such information or findings to the Client.
By signing below, I hereby consent to the search and disclosure of my criminal record information, exclusionary lists, driver’s license record, credit information, sex offender registry check, social security number validation and search, educational records as defined by the Family Educational Rights Privacy Act and employment verification.
